Evidence-Based Approach

Assertive Community Treatment (ACT) and Community Treatment is one of the most documented and effective models of integrated community care for people with serious and persistent mental illnesses. This approach is intended to help people “get the most out of life” while reducing hospitalization, incarceration, and homelessness.

Community-Based Services

Our clients receive a full range of medical, mental health and rehabilitative services in numerous locations. In fact, the ACT team delivers 80% or more of the services in a community setting, outside of our office setting. Meeting locations include the client’s home, homeless shelters, schools, place of employment, jails, churches, parks, on the streets or wherever is determined to be a clinically-appropriate setting. Such alternate settings encourage participation and prevent dropout.

Multidisciplinary Approach

  • Provides direct treatment to people with severe and persistent mental illness;
  • Provides most of the medical and rehabilitative services that the client needs, which limits the number of necessary outside referrals that would be required;
